All requests for searches and reinstatement begin with the completion of a program intake form. Because of the importance of acquiring accurate and complete background information, intake forms are specific and detailed. There are six different intake forms. The following is a description each:

		Birth Parent Intake To be filled out by birth parents who wish to search for an adult child who was adopted or placed in long-term foster care.
		Adoptive Parent Intake To be filled out by adoptive parents who are requesting support services for a legally adopted adult child or requesting a search for the birth family of a legally adopted minor child or adult child.
		Birth Sibling Intake To be filled out by a birth sibling (brother or sister) who is requesting a search for an adult birth sibling who was adopted or placed in long-term foster care.
		Adoptee Intake To be completed by an adult adoptee who wishes to access support services or request a search for birth family.
		Maternal Extended Family To be completed by maternal extended family (aunt, uncle, cousin, etc.) who wish to search for a family member who was adopted or placed in long-term foster care.
		Paternal Extended Family To be completed by paternal extended family (aunt, uncle, cousin, etc.) who wish to search for a family member who was adopted or placed in long-term foster care.
